"Lack of collaboration between different healthcare providers"

My husband received day surgery yesterday. He said his experience at Queen Margaret was good and all the staff on the day were brilliant. However, the aftercare plan (i.e. seeing GP/Nurse for dressing check) not great. I really feel this should be arranged by the hospital team before a patient is discharged. Leaving the patient who is in pain and uncomfortable to try and get through to their GP isn't ideal. Especially when the GP is short staffed and takes a long time to answer your call to then be told there are no appointments and given the number for the district nurse team. Then it took hours to reach the district nurse team to be told they only see housebound patients.  

Please can management take time to consider the aftercare of patients and work on better collaboration between different healthcare providers.

Many thanks for taking the time to read this and a big thank you to the staff who took care of my husband yesterday, this is by no means a complaint. Just an observation and recommendations for better patient experience.
