My mother-in-law was recently transferred to a nursing home from the Ulster hospital. The social workers failed to tell the nursing home her arm would be in cast and couldn't walk stairs as her mobility is very limited, the nursing home said they wouldn't have accepted her if they had known, as it's the only room they have available which has stairs and no toilet.
One would presume a social worker would carry out, communicate and assess to ensure the room is accessible and at least pass the information on to the reason why she was in hospital in the first place was she fell and broke her arm.
What rehabilitation were they planning if they didn't even know her arm was in cast? This is totally unacceptable, she wasn't even medically fit to be discharged and is discharged to somewhere that is not fit and does not meet her needs, and to top it of they are expecting her to pay, it's absolutely disgusting and needs addressed ASAP.
This was a total failure of care.
